Shortly after a youth is admitted to a Qualified Residential Treatment Program (QRTP) on a 老澳门六合彩开奖直播 campus, our staff begin the process of preparing the youth and their family for the next steps that will follow residential treatment. QRTP Aftercare provides ongoing services for youth and families to help them continue their path to healing.

QRTP Aftercare provides discharge planning beginning within 72 hours of the youth鈥檚 admission into the program.听 Goals related to discharge and aftercare are developed and presented in the first treatment plan and in every subsequent treatment plan. The goals include clear and measurable objectives which may change during the youth鈥檚 time in the QRTP to reflect progress toward achieving the goals.
Once a youth is discharged from the QRTP, the Aftercare services they receive will include the following:
- Casework contacts with the youth, which will consist of at least two face-to-face contacts per month. At least one of the contacts must include the family or permanency resource for the youth.
- For youth 18 years or older, face-to-face contacts may be conducted via video call if the youth and the local Department of Social Services agree to such an arrangement and the youth has the necessary technology to allow for such communications.
- Meetings with the permanency team, including the family, no less than twice during the aftercare period, including once no less than 30 days prior to cessation of aftercare activities.
- Comprehensive aftercare services identified in the last treatment plan prepared by the QRTP staff.
The length of QRTP Aftercare will vary depending on the needs of the youth, but will often last approximately six months following discharge from residential treatment.

The program serves New York State counties including Allegany, Broome, Cattaraugus, Chemung, Chenango, Cortland, Erie, Genesee, Herkimer, Jefferson, Lewis, Livingston, Madison, Monroe, Niagara, Oneida, Onondaga, Ontario, Orleans, Oswego, Schuyler, Seneca, Steuben, Tioga, Tomkins, Wayne, Wyoming and Yates.
